Time-Less Voices, LLC Death Doula Services Agreement Parties:

    This Agreement is entered into on date: between Time-Less Voices, LLC, located at Stuyvesant Heights, Brooklyn, New York, and the undersigned Client, Executor, and Caregiver(s) , located at

    Services Description:

    Time-Less Voices, LLC provides Death Doula services encompassing the following:

    • Consultant Services: Providing guidance and support regarding end-of-life planning and decision-making.

    • Direct Care: Offering compassionate care and assistance to clients nearing the end of life.

    • Legacy Work: Assisting clients in documenting and preserving their life stories and legacies.

    • Advance Care Planning: Facilitating discussions and documentation of medical wishes and preferences.

    • Companion Support: Providing companionship and emotional support to clients and their families.

    • Educational Workshops or Death Doula Cafe: Hosting workshops and discussions on end-of-life topics.

    • Flexible Availability: Being available to clients based on their needs and schedules.
      Companionship and Emotional Support: Offering emotional support during the end-of-life journey.

        • Terms of Service:

      1. Scope of Services:

        2. The Provider agrees to provide the Death Doula services outlined above.
      2. Payment

        2.1 The Client agrees to pay the Provider the following fees for the services rendered:
        Payment Amount: [Specify payment amount and frequency, e.g., $ per hour/session ]

        Payment Schedule: [Specify payment schedule, e.g., monthly, per session, upfront payment, etc. ]

        Payment Method: [Specify accepted payment methods, e.g., bank transfer, credit card, etc.]

        2.2 Any additional services requested by the Client beyond the scope of this Agreement shall be subject to negotiation and Agreement on separate fees and terms.

      3. Scheduling and Booking

        4. 3.1 Both parties agree to schedule service sessions in advance. 3.2 Scheduling arrangements may include in-person visits, video calls, phone consultations, or other mutually agreed-upon methods of communication and interaction.

      4. Cancellation and Rescheduling

        5. 4.1 The Client agrees to provide at least [Specify notice period, e.g., 24 hours] notice for cancellations or rescheduling of service sessions. 4.2 Failure to provide timely notice may result in a fee as outlined in the Payment section of this Agreement.

      5. Confidentiality

        6. 5.1 The Provider agrees to maintain confidentiality regarding all information the Client shares unless otherwise required by law or with the Client's explicit consent.

      6. Termination

      7. Acceptance
